Overview
Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities and seniors find and keep housing. Housing Stabilization Services are state plan (not waiver) Home and Community-Based Services (HCBS).
Eligible Members
Be on Medical Assistance (MA)
Be 18 years old or older
Be assessed to require assistance with at least one of the following areas resulting from the presence of a disability or a long-term or indefinite condition:
Communication
Mobility
Decision-making; or
Managing challenging behaviors
Managing challenging behaviors
Have a documented disability or disabling condition, defined as one of the following:
A person who is aged, blind or has a disability as described under Title II of the Social Security Act.
A person with an injury or illness that is expected to cause extended or long-term incapacitation.
A person with a developmental disability (or related condition) or mental illness.
A person with a mental health condition, substance use disorder or physical injury that required a residential level of care and who is now in the process of transitioning to the community.
A person who is determined to have a learning disability according to policy adopted by Minnesota Department of Human Services (DHS);
A person with a substance use disorder and is enrolled in a treatment program or is on a waiting list for a treatment program.

Be experiencing housing instability, evidenced by one of the following risk factors:

• Helping with rental applications, housing searches, and landlord communication.
• Supporting individuals in understanding their rights and responsibilities as tenants.

• Connecting individuals with community resources and services.
• Developing a housing stability plan tailored to individual needs.
• Providing skills training to ma
• Teaching budgeting, financial management, and lease compliance.
• Offering employment assistance and job readiness support.
• Supporting individuals with daily living skills needed for independent housing.
• Employment & Financial Stability
• Assisting with finding and maintaining employment.
• Helping individuals increase financial stability to support long-term housing success.

Financial assistance
Helping individuals cover rent, utilities, or other housing-related costs.
Management
Ongoing support to address issues like budgeting, maintaining a household, and navigating social services.
Support for special needs
For individuals with mental health, physical health, or addiction challenges that might affect their ability to keep housing.
Preventive services
Assisting individuals before they face eviction, helping to address problems like job loss, relationship issues, or other challenges.
Transitional Housing Services (DHS MN)
Temporary housing
Providing a safe place to live.
Management
Ongoing support to address issues like budgeting, maintaining a household, and navigating social services.
Life skills training
Teaching skills such as budgeting, job readiness, and parenting, which are crucial for maintaining housing long-term.
Employment assistance
Helping individuals find work or improve their employment situation.
Consultation Housing Services (DHS MN)
Consultation services are typically offered to help individuals or organizations better understand housing options, resources, and services available to them. These consultations may provide guidance to landlords, service providers, or individuals in need of assistance with housing-related issues. The focus of consultation services is to give expert advice and support in navigating housing challenges.
Housing referrals
Directing individuals to appropriate housing resources or services.
Legal advice
Offering guidance on tenants' rights, eviction prevention, and lease agreements. Support for service providers .
Housing Transition Services helps a person plan for, find, and move into housing and may include:
- Helping the person think about preferred housing
- Helping with the housing search and application process
- Developing a budget.
- Understanding a lease
- Helping to pay for moving expenses up to $3000 annually
Housing Sustaining Services helps a person keep their housing after they have moved in and may include:
- Education on tenant-landlord rights and responsibilities
- Coaching to develop relationships with property managers and neighbors
- Training on how to be a good tenant; lease compliance
- Problem-solving to maintain housing stability
Housing Consultation Services helps a person who does not have Medical Assistance (MA) case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community and may include:
- Developing a housing focused person-centered plan based on the person's needs, wants, and goals for housing
- Helping a person make an informed choice in their housing transition or sustaining services provider
- Offering resources related to non-housing goals
- Coordinating with other service providers already working with the person
